Measure: Diabetic Retinopathy: Communication with the Physician Managing Ongoing Diabetes Care
Measure Description: Percentage of patients aged 18 years and older with a diagnosis of diabetic retinopathy who had a dilated macular or fundus exam performed with documented communication to the physician who manages the ongoing care of the patient with diabetes mellitus regarding the findings of the macular or fundus exam at least once within 12 months.

Quality Measure Documentation Workflow
1. The patient must be 18 years of age or older and have at least one face-to-face encounter with the provider during the reporting period.
2. A diagnosis of Diabetic Retinopathy (recorded by using any of the ICD9 codes listed below) must be recorded in the patient's Summary Active Problems field or encounter Assessment field:
362.01, 362.02, 362.03, 362.04, 362.05, 362.06.
3. Documentation to record patients that had a dilated macular or fundus exam performed with documented communication to the physician who manages the ongoing care of the patient with diabetes mellitus regarding the findings of the macular or fundus exam at least once within 12 months. This can be documented by inserting the shortcut code "DiaRetCom" in the encounter Plan field or Summary Interventions field and using code: 0089a.
Measure Calculation Details
Numerator Calculation:
The numerator for this measure is calculated based on the following:
1. The number of patients in the denominator that also have documentation of a dilated macular or fundus exam performed with documented communication to the physician who manages the ongoing care of the patient with diabetes mellitus regarding the findings of the macular or fundus exam at least once within 12 months. This can be documented by inserting the shortcut code "DiaRetCom" in the encounter Plan field or Summary Interventions field and using code:
- 0089a: Communicated with the physician managing ongoing diabetes care.
Denominator Calculation:
The denominator for the measure is calculated based on the following:
1. The patient must be 18 years of age or older and have at least one face-to-face encounter with the provider during the reporting period.
2. And have a diagnosis of Diabetic Retinopathy (recorded by using one of the ICD9 codes listed below) must be recorded in the patient's Summary Active Problems field OR encounter Assessment field:
- 362.01, 362.02, 362.03, 362.04, 362.05, 362.06
