New CHA2DS2-VASc Score

CHA3DS2-VASc Scoring

SOAPware has added a feature that will allow the user to insert and calculate the CHA2DS2-VASc score.  This is a clinical prediction rule for estimating the risk of stroke in patients with atrial fibrillation (AF).

Inserting CHA2DS2

To insert and calculate the CHA2DS2 score for a patient, open the patients chart and the Vital Signs chart section.  Right click on the Vital Signs tab and select New > General > CHA2DS2.

Scorecard

The CHA2DS2 score will be added into the patients chart. This interative score will be based on several items that are recorded in the patients chart, including the history of diagnoses that are recorded in the patients Summary Active Problems or Inactive Problems. The score will pull the patients age and birth sex from demographics.  Based on these factors, the Total Score, Risk Group, Stroke Risk, and TIA/Systemic Embolism will be calculated.  

The above interactive score will attempt to pull this data from the patients chart, however the provider can choose to overwrite any of the fields or add comments to any of the fields to calculate the appropriate score.  Each recording can be signed off using the Sign off button located in the top toolbar.

Transfer to SOAP Note

The user can also transfer the score to the patients SOAP Note by clicking the Transfer to SOAP Note button.
