CMS 1500 PDF Editable

The new CMS 1500 form can be edited within the Claim Viewer to add/change information that is payer specific, but  isn't documented or pulled from the patient's account. These edits will only apply to paper claims, and must be printed before closing the print view. Editing within the viewer will not change the visit or patient account information, and will not be saved once you close out of the print preview.

The claim must be printed in the Ready To Submit section to prompt the confirmation dialog to change the claim status to Submitted, and remove the claim from Claims Manager.

  1. Go to Claims Manager, process claim as usual by Rebuilding, if applicable and Scrubbing to move to the Ready to Submit section.
  2. Click the PDF icon to open Claim Viewer.
  3. Form option defaults to the CMS 1500 (02/12) form. If using pre printed CMS 1500 (08/05) forms, select the 1500 Claim Form 08/05 Version button.
  4. Click inside block to activate the field as shown in the screenshot., and begin typing necessary information into the field.
  5. Click Print button.
  6. Close the print preview.

7.    Select Yes to change the claim status to Submitted, and remove the claim from Claims Manager, or No to keep claim in the Ready to Submit status. Claim will remain in the Ready to Submit section.
