Access to 08/05 CMS 1500 Claim Form
We have had some requests to be able to continue to print the old CMS 1500 form in the Claims Manger instead of printing only in the Chart Billing Statement section. In order to get this feature included in this release, the option to print the entire (08/05) form using a color printer is no longer available, and only pre printed forms can be used. The print preview also appears to be out of alignment, but it but will align properly with the pre printed (08/05) forms.
The option to print the CMS 1500 (08/05) form in the patient account Claims tab on the fly is also available, however, we recommend doing this in the Claims Manager to properly document claim, and change claim status. The claim must be printed in the Ready To Submit section to prompt the confirmation dialog to change the claim status to Submitted, and remove the claim from Claims Manager.
- Go to Claims Manager, process claim as usual by Rebuilding, if applicable and Scrubbing to move to the Ready to Submit section.
- Click the PDF icon to open Claim Viewer.
- Form option defaults to the CMS 1500 (02/12) form. If using pre printed CMS 1500 (08/05) forms, select the 1500 Claim Form 08/05 Version button.

The text in the Claim Viewer appears to be out of alignment, but will print properly when using the pre printed CMS 1500 (08/05) forms.
4. Click Print button.
5. Close the print preview. Process Claim dialog will open.
Submit edited paper claim and Remove from Claims Manager
6. Select Yes to change the claim status to Submitted, and remove the claim from Claims Manager, or No to keep claim in the Ready to Submit status. Claim will remain in the Ready to Submit section.
