This error could be received in the Billing Statement when attempting to post new charges to the patient account, and one of the procedure codes in the SOAP note Plan section does not have a related DX.
You might try:
Relating the diagnosis code to the SOAP note and recreating the billing statement.
If the chart has been signed off you will need to delete the CPT/HCPCS code from the Billing Statement and then post to billing. Add the deleted code along with the diagnosis to the New Charges tab and then post to patient account.
