3. Maintain Problem List

This lesson will demonstrate the steps necessary to meet the Meaningful Use Core requirement for maintaining an up-to-date problem list of current and active diagnoses.

Version: SOAPware version 7.0.2 or later must be used

*Note: To determine the version of SOAPware being used, click on Help > About SOAPware. This will open a window which will show the version and build of SOAPware currently in use.

 

Measure Criteria

Objective: Maintain an up-to-date problem list of current and active diagnoses.

Measure: More than 80% of all unique patients seen by the eligible provider must have at least one entry or an indication that no problems are known for the patient recorded as structured data.

Exclusion: No exclusion.

Measure Calculation

Denominator: Number of unique patients seen by the EP during the HER reporting period. In SOAPware, this is the number of unique patients seen by the provider that have a face-to-face SOAP note during the reporting period.

Numerator: Number of patients in the denominator who have at least one entry or an indication that no problems are known for the patient recorded as structured data in their problem list.

Threshold: The resulting percentage must be more than 80%.
